In the patient's HEENT exam, it says patient has slight redness along the (S/L umbral or umbra) on the left and negative on the right.

Her pupils are equal and reactive to light and accommodation.  She does not have any thyromegaly or neck lymphadenopathy.  Nasal, she does not have any boggy or erythematous nasal turbinates.  She does not have any "acopalus stoning".  Yess....this is an ESL :)

For an endo note, in HEENT:  "No ocosinoas". Does anyone know what that might be? I spelled it like it sounds. Thanks.

TI cound not reproduce any pain in the epigastrium or in the subxiphoid area.  There was no s/l bulbar-igmea or abnormal bowel sounds.

Doctor dictates:

"I think the largest of her multiple breast nodules should be ultrasound core-biopsied in order to exclude the possibility of a Fah-LOI-dees," although given their size, this is unlikely, but given that they have increased in size, it may be possible."

The maternal surface is composed of the usual (sounds like caudalidens).  I can't seem to do a successful Google on this. Thanks in advance for your help. 

She still however has the heel pain. The x-ray done last time showed calcifications in the s/l tendanocrastronimus.

Left TM is still slightly red.  There is no drainage or discharge present in the canal.  No crusting.  No pain of the pinna or __tagus__. 

The HEENT exam is positive for tenderness on tapping around the right eye, and there is definite tympanic membrane retraction noted bilaterally, with prominence of the s/l inca and stabies on this exam.

HEENT:  Early chalazion is present.

Chalazion:  A chronic inflammatory granuloma of a meibomian gland.  Syn:  Chalaza, tarsal cyst.  A sty.

Mucosa is moist and unremarkable except for ??__________. It kind of sounds like pallored but he goes into his next sentence so quick it's hard to know.

s/l "little ann grojy", no dizziness or vertigo.

HEENT:  Neck is supple, no thyromegaly, no carotid bruit. Throat is normal. Nose is congested. She has some s/l trinal tenderness.

Need help with above term...thnks

HEENT:  She wears glasses.  She does dye and talk with a speech impediment due to a childhood illness. 

This is a fairly clear dictator who has never mentioned dye in this context before.  Does anyone have a better idea what this might be?  Thanks. 

under HEENT examination the doctor states -

Extraocular muscles intact.  Bilateral "turigeum".  Fundi not visualized.

This Doctor is repeatedly using a phrase I've been struggling with today.  I think I may have 'decoded' it.  Does this make sense?

HEENT: photoscopic exam is benign.
