I don't mean to be hateful at all, see my previous post. Reading her sentence I knew immediately that the word she was looking for was trachelectomy. Please, please, please research your words. Please learn that "tonsillectomy" is never under any circumstances used in any OB/GYN surgery. Notice the patient was "post hysterectomy"...there's your clue....OB/GYN.

Is dictator ESL? I get this garbagola all the time...they'll say phlebo for blood pressure and the ilk............using an incorrect word....because they cannot remember or do not know the proper word and so they GRAB what they think...lots of ESLs do this...in my experience....though I will say some are great dictators too...had a bunch of GREAT dictators today of which 3 of 4 were ESL and good dictators. There IS hope....*lol*

Milton, no ulterior motive here. I'm also not a cynic, so perhaps it's best I just graciously back away from this board. You've already answered my question. This is not a board for the likes of executives. The information flying around is inaccurate, and that's the way it will remain. Since most people are smart enough to go to the source when seeking verifiable info, I'll trust the general population to be sensible enough to contact the source if and when in doubt about what they're reading. Sorry to have disturbed you.

You're using the quotation marks at the end, which means Google is searching for that exact phrase. Take the quotation marks out, and you'll get it.

Since the definition of hydrosalpinx is "The accumulation of serous fluid in the fallopian tube" I would say that is a condition and not something that can be made into a plural. You wouldn't say "He has gouts", you would say "He has gout in both feet".
So, I'd say something like "There was hydrosalpinx in both fallopian tubes" when he tries to make it plural.
That's my take on it, anyway.

Typing in all caps is considered shouting. Who wants to read a post where the person screams at you the entire time? The majority of users simply ignore those posts, and any valuable information is lost. Please do NOT use all caps, you will only be perceived as ignorant, inconsiderate, and lazy.

Assessments need to be without abbreviations (unless the abbreviation is so obscure that you and no one else knows it). If not, when they are audited it will hit the fan. Remember all the middlemen who are going to read and make decisions about reimbursement, etc. based on the report. Some of them are $8.00 an hour employees and are making decisions regarding OUR health care and the treatments/procedures our doctors have decided upon. Does that scare you too?

b.i.d. and t.i.d. etc. are abbreviations from latin words. Week or weekly is definitely not latin. Type it out as "weekly"

I always use follow-up with a dash as an adjective ("follow-up appointment, follow-up call") and follow up with space & no dash as a verb ("...will follow up in 7 to 10 days"). I do not use 'followup'. I think this is correct, but maybe someone could enlighten me?

I would never have guessed. My question basically was, could it have been 205 because it sounded so high. I personally have never transcribed a blood pressure that high, and then the doc was not admitting them. That is where my confusion was. Thanks guys for your replies
The doc is probably saying it wrong.

I would put toco monitor, not tocamonitor. If you are not sure, then flag it. If you google toco monitor who will see the explanation. There is no tocamonitor.

blank it for the physician and/or facility and let them handle it, but you don't change something because you don't like the way it sounds. I have been doing this 20+ years, and I was taught that we transcribe what the physician says. Like I said, if you don't agree with it flag it for someone higher up, but our job is not to change something just because it may or may not be in someone's opinion "politically correct." People are way too sensitive these days. The facility/doctors I work for want what they say. It is not my job to change their view of the world or what they decide to include in their report, whether it bothers me or not. It ultimately comes back to them.

I wasn't trying to be smart when I asked for a reference, I was confused by all the answers above and thought you actually knew what you were talking about. Missed the mark on that one.

Staging the disorder helps physicians formulate a treatment plan and evaluate response to therapy. According to the American Society for Reproductive Medicine, endometriosis may be classified as stage I (minimal), II (mild), III (moderate), or IV (severe), based on number, location, and depth of implants and presence of filmy or dense adhesions
