my mega spell check says readmitted is okay, but you're right, it does appear odd. as far as 24-hour, that depends on wether or not it's a compound adjective. A 24-hour history, or 24-hour urine, would be correct. but, 24 hours ago, or in 24 hours is not hyphenated. hope this helps.

His left leg below the knee is S/L violaceous, red, hot, erythematous and tender to palpation.

REASON FOR VISIT: s/l mestakin-type discoloration.
HISTORY OF PRESENT ILLNESS: The patient underwent left breast modified radical mastectomy in November of 2008. She had been doing well, however she sought physical therapy/occupational therapy for left chest discomfort. She had one treatment and subsequently noted a brownish discoloration of the skin.
I definitely hear the -type discoloration just can't get the first word.
