They sometimes also do this in the HEENT exam also, not etched in blood, could be under general, HEENT, neuro, mental status exam.......


For example:


GENERAL:  The patient is a 50-year-old male with a somewhat flattened affect.


VITAL SIGNS: ....blah....blah blah
