would * muscular * distention make sense? What muscles? I have never heard of muscles being distended in a neck exam. That may not mean anything as I am sure I have not heard of everything in the world of MT.
Could it be * vascular * distention? as in no vessels being distended, such as the jugular?
oh well, you have probably sent off the report by now.....

I googled the image for Coversite, and on the packaging, it said CovRSite. I suppose either spelling would probably be okay, but I would feel safer using what is on the box.

Must be hyper then if they're putting on aspirin. Glad you figured out what to do. Just wanted to let you know that my oncology book does have hypocoagulable as a word and every place probably has their own terminology for things, bleeding studies, coagulopathy workup, etc. etc. Let us know how it turns out if you find out...

On his hearing test, the patient could not hear frequencies at 1000-2000-4000 with his right ear at 40 decibels. He could not hear 1000 with his left ear at 40 decibels. However, during the whisper test, he was able to hear three items in each ear from at least 7 feet away.
I have no idea how to type the 1000 2000 4000. This is on a physical exam. Thanks for any help!!

I would do it this way...
CLINICAL IMPRESSION: 1. Fever. 2. Bronchitis. Rule out a right pneumonia. 3. Chronic lymphocytic leukemia. 4. Atrial fibrillation. Rule out arrhythmia.
The reason I do it that way is because way before, we used to line Rule out under let's say Atrial fibrillation, yielding 2 lines. It used to be done that way back in the early 90s. Now it is not done that way. So, I give the impression which would be atrial fibrillation and begin a new sentence....because they have to rule out an arrhythmia. I don't think it is etched in stone, so either/or would be fine. *S*

Document: United States Patent 5709459
Abstract: A luminaire for use during a medical procedure utilizes a single optic fiber for providing a beam of light having an initial beam width and an initial beam energy. The width of the beam is adjustable without substantially decreasing the beam energy, and the beam of adjusted width can be directed onto a surgical field. A headband for mounting a luminaire is formed of a perspiration-absorbent cloth strip, has hook-and-clasp engagement, and includes a luminaire mount.
