Don't know first S/L. What comes before that?

Second S/L may read: . . . centrally with facet Osteo??
(osteoporosis or osteophytes, maybe?).

Third s/l may be significant canal stenosis, maybe even central canal stenosis. (Wish I could listen.)

Also "cannot" is one word always!

Please relisten and see if the sentence is a little clearer, and then I'll be back.

if the doc dictates "eleven thirty", then you would type it as 11:30 and make sure you use the digital format throughout the remainder of the report to keep consistency.  (i.e.: The patient had a palpable lump at 11:30 in the right breast and at 1:00 in the left breast.) Do not type 11:30 o'clock (even though some will actually dictate it that way).

If all locations are on the "whole" hour then you do use the format of 11 o'clock.

A 22-gauge long spinal needle was inserted directly into the (s/l-THEP)  joint.  It's not hip.  I'm sure of that.  It sounds like it starts with a TH or an F.  This is for a fusion for a spinal fracture.

THANKS!

Didn't you ever get this?
Subject: Didn't you ever get this?

The reason it didn't occur to people was that you didn't give enough information to let them know it was part of the physical exam, either HEENT or neuro exam. When clues are provided, people are happy to help. Otherwise it's a lot of effort on their part and none on the person asking the question.

you must not have read her post, which states:

"I agree it looks weird, but "appropriate for gestational age" is a compound modifier for "infant." Weird or not, that's the way it is. My opinion, if the correct way looks too "weird" for you and you don't mind getting pinged by QA, then use no hyphens at all because "gestational- age" definitely should not be hyphenated."

Another option, if this is not a verbatim account, would be to re-cast the sentence.
