The patient is moribund. She is unable to speak at the current time and does not respond to sternal rub. "Chiller" venous veins appear normal. Site of the PEG tube is normal without any evidence of exudates.
She is 92 years old and comfort-care only. Here is some past history:
Past medical history is significant for progressive dysphagia leading to frequent bouts of aspiration secondary to a prior cerebrovascular accident in 1997, recent admission for probable aspiration bronchitis/pneumonia, history of hypertension, history of thyroid replacement therapy, history of possible dehydration. She has a prior history of cataract, prior history of gallbladder surgery, recent PEG placement in 11/2005, status post TAH-BSO at age 40 and eyelid surgery in January 2002, history of chest pain admission in February 1996, history of cerebrovascular accident of the right internal capsule in June 1997, and history of TIA in August 2003.
