If they said *a total of 10 cc* then it would be was.....a total was injected. However, if they say *approximately 10 cc* it would be were injected as it is more than 1 cc.

Per that text, the "5 mL" (but not the "lidocaine") becomes the Subject, and the Verb should be in plural.
