I would say "cataract surgery to bilateral eyes" is correct. I guess (to me) I look at bilateral as being interchangable with "both"... I wouldn't say "cataract surgery to both eye", it should be "both eyes". Of course "both" can't be interchanged with "bilateral" on all occasions, but (right or wrong) this is usually how I decide which way I type it. But, that's just me... maybe? :)
