When patient's have seizure disorder or passing out episodes, etc. most states want physicians to fill out a "Confidential Morbidity Report" (or name similar to that) and send to the DMV letting them know the patient has been diagnosed with condition that can cause loss of control of the vehicle -- did doctor say he counseled patient on not driving until seizures under control for x-amount of months or something like that?

but before the signature. Makes it easier. We typed our reports on sticky paper and that way it did not get separated from the main report etc. But I did not do BOS. nm

Is the account verbatim? Abbreviations are not acceptable (most places) if they are used in the diagnosis headings, but are okay if in the body of the report, unless the facility has requested otherwise.

I worked in radiation oncology and then helped develop surgical oncology for a local hospital all during the 80s and got very familiar with all the terminology (but admit that I have trouble with all the new combo drugs that are always coming out).
It gets easier the longer you do it!!!

In an operative report for a cervical laminectomy and fusion, the doc says "the screw holes that had been placed were placed using the s/l Howard On technique with 30 degrees lateral and 15 degree cephalad angulation from the center-center position.
Maybe the word I'm looking for is Howard An because it sounds like he say a-n right after he says the word technique, as if he's spelling An for me. I can't document this anywhere in my Google search efforts though.
