what is the correct way to type numbers

0, 1, 2, 3, 4 ... or 0 to 10 spelled out, and then starting at 11, type numerically, 11, 12, 13, 14 .... I was taught that a long time ago, but I'm not sure what the current rule of thumb is. Thanks!
numbers - type out versus arabic
Question: What is the rule of thumb: In a report I know it is in medication and measurements like 5 mg and 5 inches. But what about in dictation? 5 weeks or five weeks, 5 children or five children. I know BOS changed their policy on using more arabic numbers instead of typing out. Can someone advise what is the preferred way? Does it apply something to things or objects versus units of time like 5 weeks or 5 days? Thank you.

p.r.n. at beginning of sentence
Subject: p.r.n. at beginning of sentence
I type on a verbatim account. In the medication list when they dictate p.r.n. Tylenol, for example, I have been typing P.R.N. Tylenol. I cannot verify this in the BOS. Many people type P.r.n. Tylenol, which I think is not proper. What do you think is correct? Thanks.

Subject: How do you write pH at the beginning of a sentence?
I have a doctor who says, "pH is within normal limits." I've been adding the word "the" before pH just because I don't know if it's supposed to be capitalized or not at the start of a sentence. Yeah, I know, he's not being gramatically correct, but verbatim, you know.
Thanks!

Subject: I would type it under PLAN and type what is dictated.
I have had that happen to me before, and I was told by QA to not move things around in the report, maybe that is the same for you, but if I were typing this report, I would type it in the order as is dictated, even if that means typing it under the heading PLAN. Also, his vitals were probably taken at the end of the exam as they were repeated, so it would make sense to type them again under that heading. Hope that helps!
