Physical examination today reveals a large abrasion over the posterior aspect of his back. Otherwise, there is no external abnormalities other than the right clavicle with prominence of the midportion and the s/l spike fell in the subcutaneous tissue. tia.

When something happens to a patient after his meeting with the doctor, where and under what title do you put it on your report. I have a patient who passed out while making a follow- up appointment and needed to be sent to the ER. There is a whole paragraph dedicated to this. Where should I put it and under what title?
