ASSESSMENT:



  1. Type 2 diabetes, slightly suboptimal control despite institution of long-acting insulin. 

  2. Hypertension, controlled.

  3. (s/l hypereristhemia), controlled.

  4. Hyperlipidemia, controlled.

PLAN:


We will continue with Glucovance 5/500 twice daily and metformin 500 mg twice daily, Benazepril 40 mg daily, Lipitor 20 mg at suppertime, Allopurinol 300 mg, atenolol 50 mg daily, hydrochlorothiazide 25 mg daily.
