I don't have a clue what this doctor is saying.  The patient has a history of ____ malformation with subsequent surgery.  Since surgery, she has had a tongue that deviates to the left.  It was crooked due to muscle atropy and ____.


The blanks sound like "guillare malformation."  It actually sounds like the doctor is saying G-R-E malformation.  At one point the G has a hard sound, but when he says it again he uses a soft G sound (like J).

Anyone ever heard of a "tongue" of intestinalized tissue.  Pnt has Barret's and is having an EGD and twice I heard what sounds like "tongue" of tissue.


Sentences are -


1.  Preop diagnosis - Tongue of intestinalized tissue extending from the GE junction.


2.  On withdrawing the scope the patient's GE junction was regular with a tongue of pink tissue extending less than 2 cm.


I can't find it.  Thanks.

Tongue-tied doc today--humulin N 15 mg s/m

I've been typing one of my usual docs today, and apparently she's having a bad RX Monday or something.  The latest one s/l Humulin N insulin 15 mg at bedtime.  The bummer for me is that I won't take a chance on a med, so look up stuff I already know.  I hate having to leave QA Markers on those I'm not 100% sure about, but I'd rather waste my time making sure (for the huge bucks I get) than to get it wrong.

I'd guess you are right that he did mean Lachman, just got carried away with the Mc part.
Iwould guess he is saying "mother tongue", or the primary language.

I'm not sure if I'm any were near the right spelling or pronunciation but it is something like pa-nen-ta-meb and Xeloda.


Here's part of the report: Status post left hepatic lobectomy, isolated segment V resection and radiofrequency ablation of five additional lesions with multifocal metastatic colorectal cancer.  She is restarting her penetameb and Xeloda.  she has also had TheraSphere treatment.

Klor-Con SA.  ?      I have heard of Klor-Con, but maybe it just sounds like "SA" after the drug name.  It is in a list of medications and the dose is 10 mEq.
