Does component make sense?

doctor lists a whole list of meds and at the end says "simvastatin 20 mg s/l 60 and and spironolactone 25 mg q a.m." What does the 60 have to do with anything? The only other thing it might sound like is "simvastatin 20 mg 6:00 p.m., spironolactone 25 mg q .m. but I have never heard a doctor say a specific hour to take a drug and it's really sort of a wild guess, so I would like to hear another opinion. TIA!

I'm have a lady with a femur fracture and he's talking about her THR. He is saying "there is evidence of a "t-con" to the component. Overall alignment appears to be satisfactory

Pt. had chest pain while walking...then the Dr. dictates " he felt like he was hit in the chest. There was a 'somewhat phoretic component to that'....." any ideas? I've tried to make "diaphoretic" out of it, but I do not hear that. Please help and thanks!!!!

The nature of her chest wall spasms is not clear. The first thing that comes to mind is herpes, but this would have to be herpes syn rash. There does not appear to be a cutaneous component to it, and postherpetic neuralgia is less likely but still remains the most likely diagnosis.
