Review of x-rays shows a distal clavicle fracture with clear disruption of the medial coracoclavicular ligaments. There is elevation of the medial clavicle region. There is no evidence of healing between two ends. This is a "s/l kin" to a displaced acromioclavicular joint separation.

I don't capitalize cotton and have never been corrected, although I'm not sure if capitalized or not, but I don't think so.
