I'm doing a picc insertion note. He says "there was good s/l weigh form or way form" noted on the monitor. Any suggestions? Can't find anything. Thanks in advance!!

I was just wondering if there was a radian nerve, since QA keeps marking it as radial and then radian and then radial, (I keep getting graded on that word and just wanted to know if I was right or wrong).
