Quote from BOS sm
"Always capitalize genus names and their abbreviated forms when they are accompanied by a species name. Always lowerecase species names. i.e. Staphylococcus aureus.
Lowercase genus names used in plural and adjectival forms and when used in the vernacular, for example, when they stand alone (without a species name). i.e. staphylococcus, group B streptococcus, staphylococci, staphylococcal infection, staph infection, strep throat."
Hope this helps!
Please don't just quote ref. books
Please don't just quote reference books; I have my own, thank you, and they are all just as conflicted as yours. Stedman's, Tessier, Sloane, Google, etc., all list Moschowitz, Moschcowitz, Moskowitz and Moscowitz in roughly equal proportions. None gives an actual first name or cites the pioneering publication. Please, does anyone actually know who this procedure is named after? Is it hernia repair pioneer Alexis Moschcowitz of Mt. Sinai? Full doctor name and facility or publication title, please!!!!!
I'm sorry, that is not a proper quote. SM

1. If it is a verbatim account, you should put pee in there if he said it.
2. The patient reports, "When he tried to pee, he got weak, dizzy, hit the floor, did not feel right."
The right way if you were going to quote this would be:
When he tried to "pee" he got weak, dizzy, hit the floor, did not feel right.

I would type it under PLAN and type what is dictated.
Subject: I would type it under PLAN and type what is dictated.
I have had that happen to me before, and I was told by QA to not move things around in the report, maybe that is the same for you, but if I were typing this report, I would type it in the order as is dictated, even if that means typing it under the heading PLAN. Also, his vitals were probably taken at the end of the exam as they were repeated, so it would make sense to type them again under that heading. Hope that helps!
