I've done orthopedic examinations where "symmetrical with no swelling" was used. I know that doesn't sound like "farming," but a lot of doctors don't speak clearly and if you listen to it again with "swelling" in mind, see if it could possibly be swelling.

It sounds like the doctor is saying the patient has "collection swelling" over the olecranon bursa area. Would this make sense or seem proper? I assume he is saying there is a collection of fluid.

Should I type this as 6.90 minus 1 with swelling? Just had the same doctor with another patient coming in with labor, and she says "Cervix is 390 and 0." That should be 3.90, meaning the cervical length, right? But what about this patient of 690 minus 1 with swelling? Is that cervical length of 6.90? Thank you.

Decrease Metformin to once a day. Will probably reduce Actos to 30 mg a day to decrease the swelling of his feet.
It sounded right but is Actos going to help swelling of the feet? He said throughout the report he had swelling of the tongue. He has type 2 diabetes and sugar levels are running low. Just checking another opinion.

here is more info on the patient.
She has marked soft tissue loss anteriorly as she always has had and I can easily see, if not palpate, the humeral head beneath her skin, though fortunately it remains virtually non-painful. She has anterior instability and I suggest that she likely is subluxing or even dislocating this humeral head because of her complete absence of soft tissue.

I would do 60%. I have several doctors that will say 60 (as in ) 6-0. Clarifying the numbers so you don't put 16. They do 50, 5-0 so you don't put 15 because they sound close. It just sounds like sixty-six-zero. Put what you feel is right but I would be from experienc it is 60%
