In the history, the doc dictates "We initially saw this young man with an episode of **sid-nims-corrhea** in 2003. There is nothing else to help me figure this out, and I have tried multiple spellings. He is a very clear doctor most of the time, but I am totally stumped. Any guidance would be greatly appreciated!

Distant heart sounds. There is no S3 or S4 that I can appreciate at the time of my exam. He is not short of breath either. There are no s/l wisps or hues .
The patient does have a history of heart problems including afib, coronary artery occlusive disease, ischemic cardiomyopathy, stents, etc.

This has been a tricky one for me. Newborn female, Vater or VACTERL anomaly. In the physical exam the doctor is saying "imperforate anus, only single opening at the" what sounds like cranium "which is the vaginal opening" I have found a lot of web sites, but nothing that says the cranium as being an opening. I'm stumped!

For the migraine headaches, the patient is going to continue on the Elavil 50 mg. However she was concerned about breast feeding. The information available does show that it does cross into the breast milk, however in very small doses. Pediatric calcification is that there is potential toxicity.
Last sentence is what ASR spit out, problem is that is what it sounds like, and I'm flummoxed!!!! Help!
