He has been monitoring his blood glucose which has been between 90 and 290 on a regimen of 30 RN 60 NPH twice a day.

I would use regimen. If dictator dictates regime, and you type regimen, I don't think it would be a big issue on an audit, but you certainly would be able to appeal it. I was instructed by MedQuist years ago to use regimen.
