they chose to keep the text as it was - maybe thinking that "technically" he is not a drinker right now because he is in rehab - doesn't make much sense to me.

he never again mentions the left tonsil. Even at the end of the dictation when he says the plan is to rule out a right peritonsillar abscess. Same thing he said in chief complaint, except there he said "left". Oh well....I flagged it and I'll let QA make the change.
Thanks for the advice.
Thanks...I did think of that too...that's one of the reasons I flagged it.

Hopefully next time he says it he will either say it more plainly, spell it, or whatever. I will post it when he does.
Thanks very much to everyone for their help!

NWMNMT:Good grief, good grief, good grief..If this is the case, the doctor should also explain
Subject: NWMNMT:Good grief, good grief, good grief..If this is the case, the doctor should also explain
the meaning in an ADDENDUM.
How do you know what the doctor meant? GOOD GRIEF, are you the know-it-all (hyphens must be put here, as it is used as a noun!) on this board? Get over yourself!
Who understands what it means without an explanation?
The MOST obvious is apparently like I understood it:
5 packs in 1 year.
Maybe the doctor really meant this, 5 packs per year, so essentially a nonsmoker.
GOOD GRIEF, get over yourself!

of "stable" seemed like that might be right. Slowing it way down, I can clearly hear "currently appears acceptable." You steered my thoughts in the right direction. Thank you again.

I don't really have any suggestions for your missing words, but I do have a few general suggestions.
Are the terms in parenthesis the ones you don't know? If they are, put s/l in front of it (s/l= sounds like), and then do your best to type what you are hearing phonetically. Sometimes it also helps people figure out the missing words if you can share as much about the physical condition of the patient that you know.
Good luck with your training!
Thanks for the suggestion. sm

It was really unclear.
It may have been trapezius, but I looked at it from every angle I could, thinking he may have been saying TM - for teres minor --
So I finally gave up and did the safe thing...QA!
Thanks.
my suggestion.

The patient had something in his eye. The eye was numbed with ophthalmic phenacaine, and also, the eye was stained with fluorescein and checked with blue light.
Thanks for your suggestion

I think that is what he means but is clearly spelling with a P. Guess this is one for QA. Too bad for me that he mentions it several times in the report.
Thanks for the suggestion!

You are probably right and the doctor is misprouncing the word. I still hear a K and X sound so I shall leave a blank just in case being that I do not have a firm diagnosis to match it up with. Thanks for your help!

I really appreciate. I have been banging my head against the wall trying to figure this one out. It sounds like the "five hood" could be one word. Anyway, thanks again!
