Points taken off...

Wondering if anyone is still up. Need an answer before I go to bed. Please??? Where does it state in BOS, or anywhere, that the expanded form of DNR is DO NOT RESUSCITATE, versus Do Not Resuscitate, unless stated in the client profile? QA took points from me on this one, and I want to complain to my PS (If I'm not wrong of course).

Thanks for your help!

    Atacort for blood pressure? sm
Subject: Atacort for blood pressure? sm

The doctor clearly says Atacort for blood pressure problems and even spells it. He's dictating the history given by the patient. It's a psych report, so not relevant to other areas of the report. I found Atacand, but they don't even sound or look alike.

I'll probably just flag it, but any help would be appreciated.

The doctor clearly states: Blood pressure in the field was 180 over palp, later 112/124, and then later 188 over palp.

Does the 112/124 sound correct? It is clearly stated, but I think he may be stating it incorrectly. Has anyone heard of a reading like this?

    Thank you.
