The company MedicAlert (1 word) has a trademark emblem/bracelet; that is the most common one used. But, there's an org in Canada, Med-Alert, that supplies medical alert bracelets, and another, 911MedAlert, that I found. If any of that helps! :)

The only help I can give you here (very confusing)
Subject: The only help I can give you here (very confusing)

is that "symmatric" should be "symmetric," a very common term.

Are you taking a test?

extremities (check your spelling)

Also your "Intact relflexy in purse" may be "intact reflexes in pursestrings." Difficult to determine without listening myself.

They usually say "normal sensory examination to light touch" not "a light touch."

I have a dictator that dictated in two different reports.. "No CVA angle tenderness." and the same dictator in another report states "No CV angle tenderness." I am confused as to which is the correct way ?

Always go by your company policy and produces. My company states:

"when the manufacturer uses all capitals, use an initial capital only so as not to give undue attention to the term in a medical report..."

When is Staphylococcus not capital? This is confusing me.
I always thought Staphyloccocus was capitalized until on my QA I was told different, but can't find the reason and it's been a while since this happened.  Now it bothers me not knowing how to determine whether I'm capitalizing it when its not supposed to be.  Can you help me with this?  Thanks
