is a mix up of saying Mucomyst and acetylcysteine
(Mucomyst is N-acetylcysteine). I could not document a US classification; only in Canada. The rest of the statement should be 100 mg per meter squared written as
100 mg/m2
I would suggest leaving a blank if you cannot document.

I'm not sure if I'm any were near the right spelling or pronunciation but it is something like pa-nen-ta-meb and Xeloda.
Here's part of the report: Status post left hepatic lobectomy, isolated segment V resection and radiofrequency ablation of five additional lesions with multifocal metastatic colorectal cancer. She is restarting her penetameb and Xeloda. she has also had TheraSphere treatment.

Klor-Con SA. ? I have heard of Klor-Con, but maybe it just sounds like "SA" after the drug name. It is in a list of medications and the dose is 10 mEq.

First sounds like Aralidide. Possibly a thyroid medication? Doctor did not state what it was for. Second sounds like "Loptipain"? The tape may have cut out the beginning of the drug name. It is for blood pressure.
