What you have above is an appropriate term. Maybe (probably) you need to add the term to your spellchecker.

Retinae cream, Ampicillin and sodium sulfacetamide.  These are medications that were stated the patient used for acne.  I am not sure if my spellings are correct. I have seen Retinae or Retin-A and the ampicillin I have seen with a capital A.

Acne medications to include "batanay" and "benzoquine" wtihout relief.  I am not sure if these are over the counter or prescription but I cannot seem to quite match them up and it is just "tickling" on the edge of my memory.  I must need another cup of coffee!

Past Medical History:  Hypertension, rheumatic fever age 12, osteoarthritis of the knees, temporal arteritis.


Medications:  Lexapro, Topamax, Nexium, metoprolol, Caylor, Fiasol, prednisone 5 mg for temporal arteritis, Neurontin 300 mg for controlling her periauricular pain after the biopsy, Xanax 0.25, baclofen 10 mg for her neck, OxyContin over the past year and a half 20 mg for her knees.

2.5 is 2-1/2 and 2.5 is the proper way to type it. It should be 2.5 mL in your sentence on your post. This is the rule for medications and recently they changed the way they want us to transcribe cc to mL, which was always the equivalent of each other and I don't know why they changed it (maybe they were bored). I understand that they think cc can be "miswritten" and "misread" as something else, but I think that should be directed at handwritten orders and documentation, not transcribed orders or documentation that is typed. The bottom line is they don't care what we think and we need to do it their way. I would never ever type 2.5 mL as 2-1/2 because it is not correct or professional to do so. Sometimes I think they just are bored and think of something to "fix."
