with this one, thanks to your answers... still hearing something like "Pradus" but instead of obturate, was saying on THE RIGHT. definitely NOT piriformis or sure does not sound like it, then goes on to gluteus medius and maximus.

Blood pressure 167/82. Heart rate is 90, respiratory rate 20, Temperature at max is 100.8. No JVD lungs and expiratory wheezes. There are some rales in the right base. Heart without ** and edema. **120, **3.4, chloride 94, ** 25, glucose 91, BUN 9, creatinine .6, calcium 8.5, white count is 5.7, hemoglobin 8.9 down from 10 yesterday, platelet count 234,000, 6 lymphs, 10 minus 1 basophiles, 7 bands, 76 segs.~~~~ after heart without--it sounds like a extremity then ssss sound. the other sounds like Sims 120. the other sounds like seven and a 3.4. the other sounds like bacum. ANY HELP WOULD BE GREATLY APPRECIATED!!!!

Begins s/l *effuncting* nausea and abnormal liver enzymes. This could be a title or description of the nausea, but I can’t find anything.
Then,
s/l *Cardiovicular*. She reports the nausea has resolved. . . . has post-nasal drip .. .cough is not constant. Reflux is under good control

Does anyone know what those 2 blanks might be? I see “glucosylceramidase assay” in the Stedman’s Lab Words book, but I don’t hear that in the dictation.

word here. Waddell's test is an exam used to determine if someone is faking pain. If the patient has exaggerated complaints of pain it is a positive Waddell's.

What is FUV1, used along with FEV1?? Never heard of it and cannot find. Also he also states FVC as being forced ventiltory capacity, which I know is called forced vital capacity; wonder if I should transcribe a non-word.

as to what a doctor MAY be dictating. Doctors make mistakes all the time. It is up to the individual MT to check out the validity of any info they ultimately transcribe. That's what I would do. Any MT who transcribes a "suggestion" should not be working as an MT. ;-)

Doctor is describing a patient's finger pain. He says:
"At her last visit she did have a maintenance interscalene block and reported several months of improvement in pain to a level of 3/10 when used in combination with her other conservative therapies. These have included Lac-Hydrin applications, paraffin wax applications, and (s/l "co-bayne") and protective (s/l "cutlets") to the left ring finger. The patient has dystonia and extreme difficulty in bending at the PIP joint from a functional standpoint. Numeric pain score is an 8/10..."
First word sounds like Cobain or Cobayne, pronounced just like the last name of the late lead singer of Nirvana. Second word sounds just like cutlets, like veal cutlets.
I have no idea at all what these two words are, and he repeats them in both the subjective and in the assessment. Can anyone save me from leaving four blanks? I'm sick and exhausted with a sick four year old at home, and I'm feeling a bit fussy and frustrated and would be SO thankful for the help.
