in my humble opinion, i think i would use the letter "O" since it sounds like they are describing the shape of the remaining fistula; that way, there's no confusion with the number zero "0" which could indicate the size of a trach tube...if you can rephrase in your account, you could use "trach site almost closed with a pinpoint opening"
