I forgot to put in commas. Some tenderness over the left trapezius muscle, over the suprascapula, radius, and also over the posterior axillary muscles.
Is that better? Thanks for your input. I think I wil leave it as radius. What do you think about taking off the "r" on suprascapular?
Yes after I sent it I realized that and also sm.

Subject: I just re-read your question regarding "inch" and realized
I just noticed that you didn't say the doc dictated inch. You asked if 3/8 could be referring to anything other than inch and I totally misread that.
The answer is yes - 3/8 could be referring to a cm measurement. Sorry I misunderstood.
But since you were unsure, you did the right thing in QA'g :)

How would you write the following:
1. "placed on D5 and a 1/2-normal saline at maintenance and a quarter."
2. and then later she says: "IV fluids, D5 and a 1/2 with 20 of KCl per liter"
These fractions confuse me. I assume the 1/2 goes with the normal saline in the first instance and maybe the "normal saline" is implied in the second instance, but how do I express these fractions? Did I do the "quarter" right in the first instance?

How about intravenous fluids were or was administered as in normal saline?
