Initially BOS dictated that we not use the possessive forms of these names, i.e. Parkinson ds and Alzheimer ds. Now BOS says to transcribe as dictated. This makes it a lot easier for us poor MTs. So, both ways are correct, depending on what was actually dictated.
According to AAMT BOS how do you transcribe cranial nerves II-XII
I just got a QA back stating cranial nerves 2 through 12. Is that correct?

I personally do not like the phrase * dash wise * The only way around it, seems to me, is to add a bunch more words like * as far as his cardiac condition is concerned * which does not sound like the dictator.

The zero after the decimal indicates the precision of the measurement. In lab values, you would need to transcribe as dictated in order to avoid "changing" the measure.
If you transcribed it as 4, the reader would wonder what you left off. A zero in other locations might be optional or up to the preference of the facility, but not this one.

Way back when, we were told to transcribe H&Ps and Consults in the present tense and DS & Ops in the past tense. However, with all the "new" rules and changes who knows anymore.
