On examination, she has a good thrill in the antecubital fossa. She has multiple small cutaneous ?vinyals? evident in her right arm and over the clavicle.

Sounds like herpes "syn" rash. Not skin, cause later dates no cutaneous component. sm

The nature of her chest wall spasms is not clear. The first thing that comes to mind is herpes, but this would have to be herpes syn rash. There does not appear to be a cutaneous component to it, and postherpetic neuralgia is less likely but still remains the most likely diagnosis.
