In front of a vowel, 'an' is used.


These are the exceptions, when 'a' is used in front of a vowel:

When 'o' as the first letter makes a consonant sound, naemly 'w', use 'a' instead of 'an'.  'Example:  a one-eyed man, etc....

I'm having a hard time with this one... sounds like the dictator is saying, "Allergies to Avelefs and Depakote." I'm having trouble with the one that sounds like avolefs or avilefs or avolephs or avilephs????? Any suggestions?

When I started in transcription there was a saying my boss would always quote and that was WHEN IN DOUBT, LEAVE IT OUT.  It's better to put a blank and have QA figure it out or even have the doctor fix it instead of putting the wrong information.

Physicians used to just list the allergy itself, in your case, morphine, but I have found that currently they are listing the reaction to the allergen, which in you case is hives.  Another example would be rash.  Morphine-hives.  Allergies:  Morphine causes hives.  In my experience anyway...
