as far as post-end versus postend.....hyphens are used for clarity and in this case I would have used the hyphen.....because it is usually end-stage renal disease....I'd give 'em post-end


If your account is verbatim and they said AV fistula give them what they said.  If you are allowed to expand, you could give them arteriovenous fistula.....

The remainder of the liver was heterogenous on the portal phase study, which maybe due to contrast bolus.  However, extension of the tumor into the right lobe cannot be excluded.  Possibility of butchiery type physiology should be considered since the IVC was slit-like.  Splenomegaly was noted along with ascites consistent with portal hypertension.  There was evidence of focal chronic pancreatitis.

I'm a little confused, the doctor dictates "He is at a point where he is disabled by the anal fissure pain, and I have recommended surgery in terms of an anal fissurectomy and lateral internal sphincterotomy.


There is no such thing as an anal fissurectomy but I swear that is what he is saying, and I would think it would be a fistulectomy but through the whole dictation it sounds like he is saying fissure.  I am a little confused with this.  Any ideas or help would be greatly appreciated.  Thanks.

Hi everyone.  I'm working on an esophagogastrojejunoscopy in a patient who is s/p gastric bypass.  He says "She is known to have developed a (s/l gastrogastric) fistula in the past...".  He has said this twice so far and both times it sounds exactly the same.  TIA for the help!
