What is the right way to transcribe an L5 compression fracture? or C5-6 herniated nucleus pulposus? Do you just put L5 or fifth lumbar or lumbar 5?


Do you put cervical 5-6 ?  Any help is appreciated. Debbie

It all depends on your account specifics, but I think some of the trend is to do so under the assessment or diagnosis sections, to make it perfectly clear, as one of my accounts has changed to that recently.  I would ask your QA about it and in which specific instances you are supposed to be using it.

Okay, so I'm an old MT.  In the diagnosis section--for example--if CHF was dictated, I've always expanded like this: 


1.  History of congestive heart failure (CHF) with acute exacerbation. 


But, I was told today by a fellow MT that the CHF should be expanded into its full form, but NOT to type the CHF in the parentheses.  This is a hospital that has no specific rules other than following "standard transcription practice," which I assume to be the BOS.


Is this something new and am I just behind the times?

since there is an (s) after fracture(s), I believe he meant a LeFort II five fractures.


Also, isn't it dysjunction?  I could be DEAD_WRONG on that one though.  *lol*
