Hello,

I am stuck on a word,  maybe two words..not sure! This is for cataract closure, the procedure is phacoemulsification and peripheral iridotomy.

"A superior corenomy was performed and hemostasis was achieved with wet-field cautery."

It actually sounds like "supear corenomy" but I thought he may mean "superior". But I have no idea what the next word should be.

Any thoughts?

ESL again

CT of the head showed frontal skull fracture, s/l falks and SDH along with bilateral frontal contusions. 

Patient with meningioma . . . 

There was mass effect upon the s/l falks (this is a CT scan of head).

I'm having a hard time with a few words I'm transcribing for an Indian doctor. The first is-- Frontotemporal parietal craniotomy. Eviquation of subdural hematoma. I can't figure out what the underlined word is supposed to be. Also, it sounds like she says they used Midol for intraoperative relaxation. Could that be correct? Thanks as always for the help.

The area was then draped off in sterile fashion.  An incision from the root of the zygoma directed anterior and medial approximately 10 cm long was made with a scalpel through the gal-ia.  The superficial temporalis fascia was identified.  The STA was avoided. 

A Pentero microscope was then brought in for the majority of the rest of the case.  Under microscopic visualization the dura was opened.  The dura was tacked anteriorly using 4-0 Nur-lons

PROCEDURES PERFORMED

1.  Right frontoparietal temporal craniotomy.

2.  Evacuation of an acute subdural hematoma.

3.  Duraplasty.

ANESTHESIA

S/L Schwann?

There is an indentation of the ventral ____ (s/l fecal or cecal) sac and the protrusion of that is the ventral medial aspect of the descending L5 nerve root.  They dont feel that it compresses the nerve root...

TIA

Patient is not intubated - is a clinic exam.

I can clearly hear the "lungs clear to auscultation and percussion" a bit after this section, so something is definitely there.

I know I have heard and typed this but here goes, the patient had knee arthroplasty and here is the sentence (it is the last word in bold) :

Postoperative complications did occur which included acute, temporary renal failure and the postoperative illius. 

Make any sense?

A drug sounds like Zedarone?  I cannot find anything even close.  Pulling my hair out.

I have this surgeon dictating 3 or so times in this esophagus/stomach resection report about the gastric artery was tied s/l sufflag and then clamped using the right angle.  The left gastric artery was then divided and the s/l stick tie.  Have any ideas what these 2 words may be?

There is some palpable tenderness over the metacarpophalangeal joint and some along the _____ (s/l hallucis paulisus) ligament.

The patient (s/l puked on a gonna) appendectomy, with localized right lower quadrant pain on the __, approximately 1 week previously, with a pathology report not suggestive of acute appendicitis. 

10 cc’s of 2% lidocaine without epi but with some ____ (s/l bicarb ? vicarb) were used for a successful  ____ (s/l ring) block.  the lateral quarter of her nail was removed without incident
