Based on the value (5.3) and the fact that it is listed with the electrolytes, could be potassium.

Patient has hypertension, has MI while admitted, anemia, tardive dyskinesia, SSS, and hypothyroidism; all the other meds listed seemed to have covered these problems. I did a google search and searched RXlist and came up with nothing. Also have the most recent version of quarterly drug index and can't find anything there either! TIA

When it comes to meds, if it's not absolutely certain I give them a well-deserved blank. sm
Subject: When it comes to meds, if it's not absolutely certain I give them a well-deserved blank. sm

If there's even the slightest doubt as to what med is being referred to (and no backup info to narrow it down to just one) they SHOULD get a blank, IMO. It's not the place for a best-guess, probably-meant, oh-it-must-be-___________, no matter how much our services/accounts would like blank-free reports at whatever cost.....

[That rant wasn't directed at you :-), but rather at the dictator and the current state of the MT position. :-)

Subject: WAG could it be Lasix 80 mg q.d. x7 days

Could this Lasix dose be correct?
Does Lasix 380 mg twice daily sound excessive?  The doctor originally dictated 20 mg daily and then went back and had me change it.  I saw something about a dose can be titrated up carefully to 600 mg daily with patients with severe edematous conditions........just not sure.  Any help would be greatly appreciated!!!

need potassium, too. I'm thinking that maybe what you're hearing is Lasix (comma), then maybe potassium chloride? Don't know about the 3rd.

Subject: that's a Coumadin dosage, not INR, wonder if the doc is getting ahead...sm

of himself about doing an INR level to make sure the Coumadin is therapeutic, not too much and not to little.  The INR level should be between 2-3 for most labs.  Sounds like he meant to say Coumadin, and may later on in the dictation talk about getting an INR level.  Hope this helps!
