I have been transcribing ER reports for years and I have never heard this term used. Anyway, I would not guess on this one. IMO, it is better to leave it blank and have QA inform you later so you can keep it on file  .

but the AAMT BOS dictates what is correct for the industry, getting its information from sources such as the Chicago Manual of Style, the American Medical Association book of style (which I own), etc. As the poster below states, Joint Commission goes by BOS and does not like "same" or abbreviations for diagnoses, impressions, op titles, etc. They will randomly choose patient charts and go through them to check things like this! It's the Joint Commission which recommends or removes hospital accreditations--a very powerful entity.
So, ask your QA what the account wants. If they want "same," give them "same." For other accounts, do it the correct way.

if that's it? This is usually dictated when they don't want to list the entire medication list so refer to the MAR that is in the chart.

EKG help - J Point

Cardio doc dictates EKG as follows:
EKG demonstrates normal sinus rhythm at 61 beats per minute with normal axes, normal intervals, J point elevation 1 and 0; otherwise, no ischemic changes.
But just not sure on how to type the 1and 0 or if he is saying 0, but sounds for sure like 1. Anyone with any ideas. This is a new one on me? Any help appreciated!!!

Someone new at this might have to look-up a lot things. Sometimes you just get to the end of your rope with researching especially when the doc isn't sounding too clear or the word is slurred. We must support one another though not tear each other down. Why can't people just go by the old saying of "if you do not have something nice to say then do not say anything at all". To come off like a know it all proves what? Absolutely nothing in my opinion. Not everyone is up on everything.

Although he does mention "near the perforation" later in the report.
My point is, docs sometimes use the abbreviations they use in speaking in their reports as well. Most of the time I will type them out, but it bugs me either way - I like to type verbatim and not add anything the doc didn't say, but if he says it wrong, what are you going to do? (Except maybe leave him a message.)
