My company has us type out 06/07/2008 as an example, because it is an account specific.

either is correct - but remember if you put May 10, 2007 - put a comma after the 2007 every time - a comma goes there - like this on May 10, 2007, the.......

If you have a date that reads something like -"LAD test 11/20/08 with the finding of etc" Should there be a comma after the date or is that just when they are spelled out like "november 20, 2008," Can't seem to find this in the BOS. Maybe just missing it somewhere.

but I think each hospital/client has a preference. I am going to be doing a different account where I work and now they want the dates as 6/30/2006. Go figure. They are trying to make us all crazy.
Are you working on your own or for a company? I would find out what the preference is if it is a company.

The nature of her chest wall spasms is not clear. The first thing that comes to mind is herpes, but this would have to be herpes syn rash. There does not appear to be a cutaneous component to it, and postherpetic neuralgia is less likely but still remains the most likely diagnosis.
