He does not check his blood sugar ,and he admits that he has not been dieting or exercising quote unquote he is simply too busy.

Any opinions on how to transcribe this one. Not a verbatim account.

Maybe this way?....dieting or exercising saying, "I am simply too busy."

It should also be noted that studies have confirmed that controlled blood pressure without the use of a thiazide does not resolve it ---is? or as? well of a reduction ---- of the long-term adverse reactions unless there is a thiazide included in the treatment.  The aera I have marked with --- is where I need help. Might have some little word/words wrong here. Sounds like doc has raspy voice while dictating!

Examination, evaluation, debridement of mycotic nail 1-5 bilateral feet. Nail plate débrided nailbed as tolerated by patient.

I am questioning the last part of the sentence and need help with punctuation there. Does the word debrided go with nail plate or nailbed as tolerated by patient?

Just need clarification, please.  Which is correct? 

a)  The left foot and ankle IS warm (one unit), or
b)  The left foot and ankle ARE warm (separate units).

Stupid question, I know, but after hearing it both ways for so long, I am now officially confused. 

I found out by calling the dr's office; PD=pelvic deficiency; LD=leg deficiency.  I think they make up their own abbreviations as they go along.

I have another one though, I may ahve to call them.  It is SNF, if you can help, please.

Hi, I am typing a new chiropractic account, need help with this word or phrase.  Sounds like (phonetically): "quad-ray-sin and bor-en" tenderness or "quad-ray-sin and lor-en tenderness," grade 2.5 on the right, grade 2 on the left.

Any thoughts??

don't know what exactly you want. 

but yes there are bloody bowel movements....and occult bleeding would be bleeding from somewhere, and in your case for your question would be the bowel...hemoccult-negative stool test would be no blood in the bowels.

The examination took place on xxx revealing degenerative disk disease at L4-L5 and L5-S1 with bullable bulging disks and at the L5-S1 level, noting spondylotic ridging on the right and left with resultant foraminal narrowing. 

Impression says patient has lumbar spondylosis, degenerative disk disease with resultant neuroforaminal stenosis and right S1 radiculitis/radiculopathy. 

Haven't found anything on the internet, in my wordbooks or dictionaries so far.  Boy, this looking stuff up really eats up time, doesn't it?

s/l prayer mere flu. She underwent an uncomplicated primary low flat transverse cesarean section. 'Prayer mere flu' was noted. Delivery was through low transverse incision.
