I'm typing for a doctor that I've only typed for a couple of times, and I'm having problems today. Well no wonder.
This is the s/l in the report that I'm listening to right now:
Patient had a salto long sammy at the age of two.
Compare that with what the previous chart revealed when I pulled it up - Salter osteotomy. The only part that is remotely like the s/l is the "salt" and the "my"!!!
If allowed, I would reword this...possibly, has not redeveloped a bleed.

I am student working on a non-small cell carcinoma report--the dr dictated patient has non-small cell carcinoma....patient underwent immediate ("sinoscopy"). I could not find anything, anywhere of a --scopy of lung tissue that the first part sounded like "sin"

He has had the episode of left side weakness in the past and findings on the MRI suggested for subacute old infarction. He denies any new symptoms and today has stated some visual symptoms of blurring but no binocular visual loss, new focal motor weakness, ***difficulty or sensory loss.

I took out my huge A&P book(from a prenursing course), and could not find muscles with a similar sound. The muscles I found near the thoracic vertebrae were trapezius, levator scapulae, supraspinatus and infraspinatus.

i.e., the phrase isn't a conjunction and doesn't get treated the same way in a sentence. So your first choice is correct. I find that a lot of docs dictate w, x, y as well as z. I always change this to w, x and y as well as z.
