Sjogren
Subject: Sjogren
Sjogren is a chronic systemic autoimmune disorder that sometimes involves connective tissue. Symptoms often seen are dry eyes, dry mouth and fatigue. Lymphs "attack" moisture-producing glands. It affects organ systems as well along joints, muscles and skin. while I suppose a Sjogren FLARE could resolve, this is otherwise clearly a chronic disorder. Look for your clues in the ROS and PMI.
Otherwise, frozen shoulder syndrome often resolves with anti-inflammatories, muscle relaxants, heat, ice and physical therapy.
Hope this helps.

Subject: surgical history s/l all-tee
Patient is being seen for balance problems. There is a lot of focus in the report on various tests and her previous tethered cord, lumbar stenosis, laminectomy, etc.
Past surgical history includes: "repair of a spinal fluid leak and (s/l all-tee) then the lumbar spine fusion."
Any ideas as to what the all-tee might be? At first I thought it was the doctor stumbling over her words, but upon re-listening, I'm not so sure!
