I have an ESL who states that the patient is allergic to Reglan, but that she is taking Reglan and just titrated the dose up to 5 mg. I have listened over and over for something that sounds close enough to Reglan to be what she said. I am sure I will wind up having to flag it, but thought I would see if you guys had any ideas? Thanks!

The doctor states the patient has an allergy to hives, then says she had a hive reaction to morphine.  Does that make sense to anyone?  I'm not sure if he is making a change or wants both statements.

Under allergies, doc says "Vita-Kadian". He mentions that the patient is on Kadian twice in the report, so I know he is not stumbling over Kadian. I will probably have to flag this, but thought I would see if anyone out there may know something I am not thinking of.

Has anyone ever heard of anyone being allergic to macrolytes?  I looked it up and it shows it is a dispersive electrode.  Wonder if it is the adhesive, but nothing in the report suggests that she would use (or has) something that would need this.  Thoughts appreciated.  Tx.
