It means no biopsy or imaging or other testing was done, but on palpation, lymph nodes felt swollen.
If you have ever gotten very sick and it turned out to be strep throat, mumps or mononucleosis, and maybe before going to the MD, your neck hurt, so you reached up and found your nodes to be swollen. Your MD would call these clinically swollen lymph nodes. :o)

If a patient has had spine surgery, the MRI is more than likely done with contrast. The contrast causes the MRI to "signal" when the contrast is bright in any area. That's what it means.

If I am looking for a catheter, instrument, etc., if the little "R" registered trademark is used, usually it is correct, because they don't go to the trouble to put the trademark if they aren't being reasonable conscientious about spelling it right.
HOWEVER....sometimes I get multiple results with the "R" and they're different. In that case, I try to find the site for the company that manufactures the item. It is usually listed on the first page of Google results.
A lot of the more well-known drugs have their own sites - like www.coumadin.com.
