It's hard to judge these things without hearing them - I went with what was closest to the sentence provided. While the patient could have been "dyspneic-looking like a patient with COPD or sleep apnea," that just didn't sit right with my gut.

I am doing an op report: Laparoscopic cholecystectomy with operative cholangiograms.  The doctor clearly says diagnosIS, although I know they do that all the time and then give numbered lists, so I know not to count on that. For the diagnosis, she says "Chronic cholecystitis, cholelithiasis."  She runs them together just like that, no pause in between or anything.


Do these go together the way I have them now, as one diagnosis just separated with a comma?  Should I have them separated and numbered instead?


I have no samples to go on and I've never done one of these before. What do you seasoned surgery pros think?

Can anyone tell me if I have the form of the word malleoli correct in this sentence.  Should it be malleoli or malleolus in this sentence.  The doc is saying malleoli, but it sounds funny to me.  Here is the sentence:
