And I am not sure if there is a rule or not either; however, I thing of it like substituting "on both sides", not just "both".  Therefore I think 'examniation of ankles on both sides' sounds like you actually have four ankles, two on each side (or ankles on both sides)


I wish I could help more, but I also do not know of a rule.
