doctor says cellulitis of the (the word I am trying to document), and then says, "spelled p-e-n-u-s."  I can't find this documented on Google that I can really confirm, and he spelled it that way very plainly.  I really believe this is wrong, but I just wanted to check to see if anyone else knows this term to be real.  I am thinking they just don't teach spelling in med school and perhaps he should just dictate and leave the spelling to me!

The dirty urine would not be found on blood work...maybe the are saying most recent lab work showed dirty urine?  The rest appears correct as you have typed.  Hope this helped!

This doctor is foreign and really mispronounces words so I may be way off but it sounds like either *chorthalidon* or *cord thalidon*.

I always spell abbreviations out such as this one that follows a vague number , i.e., few, several, many, etc. It just doesn't look right having the abbreviation in that sort of sentence to me. If you adhere to the BOS, it suggests spelling them out in this instance as well.
