PROCEDURE NOTE: The patient was prepped in sterile fashion with Betadine. He was given 1 cc of 2% Lidocaine without epinephrine for anesthesia. A sterile glove and prep was used throughout. He did have a #11 blade incise approximately ½ inch wound with a large amount of pus. This was drained and loculations were also broken up and drained. He tolerated the procedure well without complication. He was packed with approximately 4 cm of Iodoform gauze, which is ½ inch. He tolerated this well. Sterile dressing as well as KLING was placed on this. He was advised to strictly keep this dry until tomorrow when he is reevaluated.

pt is scheduled for a "colectomy takedown and reanastomosis" I guess I'll leave it as she says it. I thought maybe she could have been using the wrong term and meant "colostomy takedown" or it could have been "colectomy, takedown, and reanastomosis" like the pt was going to have another colectomy.

I know how that is. It's weird that some docs expect you to go against common rules. The BOS says use of actual numbers should be used exclusively but at times to use your own judgement. But, even common English rules say not to spell out numbers above 10. Now, capitalizing generic medicines is just plain crazy. But you do what they ask, no matter how ignorant. :-) Been there, done that.

The OP had used Google and found instances of both words being used. Sounds more like the OP was looking for some help with the proper usage of the word in the context of a report. I don't see why she (or he) caught any grief for asking for opinions on the proper term to use.
