"He sustained a laceration just below the lower lip that is through and through into the upper portion..."  I'm assuming since through and through is not modifying in this case, that I do not hyphenate, but I just want to be sure.  Thanks!

Assuming you are working in Word, you can add a button to your toolbar
Subject: Assuming you are working in Word, you can add a button to your toolbar

Go to Tools, at the bottom of the meun there is a customize button, click it. Once you are there open the Command tab.  You will see a list of categories on the left and commands on the right.  Click on the format category and you will see all of the format buttons you can add to your tool bar...  about 1/2 way down you will see the superscript and subscript buttons...  Click on the superscript and drag it to your toolbar.  I usually try to put it with like buttons, like the bold, underline and such.  The button should insert into your toolbar and now when you need to superscript a number you can just click on that button before you type it.  you do have to remember to click the button off after you have typed the superscripted character.

Hope this helps..

Subject: OHH yeah that does help. She did say

pt is scheduled for a "colectomy takedown and reanastomosis" I guess I'll leave it as she says it. I thought maybe she could have been using the wrong term and meant "colostomy takedown" or it could have been "colectomy, takedown, and reanastomosis" like the pt was going to have another colectomy.

Thanks so much for the help! And just in time, too, I'm just about to send in the report. Much appreciated!

Yeah. I know. 21 of the 25 doctors I routinely transcribe are ESL. It's challenging, to say the least. I was just hoping that someone would recognize the sounds. (I know that sometimes I can, even when reading what someone else heard.)

I know how that is. It's weird that some docs expect you to go against common rules. The BOS says use of actual numbers should be used exclusively but at times to use your own judgement. But, even common English rules say not to spell out numbers above 10. Now, capitalizing generic medicines is just plain crazy. But you do what they ask, no matter how ignorant. :-) Been there, done that.

and it's showing the sacrum, so I wonder if it's really a CT of the pelvis. Docs can be really sloppy about dictating the names of procedures, which can make it harder on billing.

Subject: Yeah, but I believe the OP did mention that research had been done...sm

The OP had used Google and found instances of both words being used. Sounds more like the OP was looking for some help with the proper usage of the word in the context of a report. I don't see why she (or he) caught any grief for asking for opinions on the proper term to use.
