Would my guess work? I just checked my Sloan Medical Abbreviations and Eponyms book and found neither "PHPI" nor "CHPI" listed. These abbreviations can be soooo difficult!

There is some calcification along the abductor tubricle suggestive of Pelic Reistreotic disease. 


Any clues?  I can barely give a spelling suggestion.
