I don't have it handy but the BOS also says to use "was" with mL, mg and the like, (units of measure).

There are exceptions like when they have a sentence stating 2 medications: "Then 5 mg of Kenalog and 5 mL of lidocaine were injected ... " Were is used there because it is a compound subject (if I'm getting that terminology right).

If they say "A solution of 5 mg of Kenalog and 5 mL of lidocaine WAS injected ..." the "was" is used because they are giving a solution, not 2 separate medications.
